Abstract

Several cases of failure of alumina ceramic hip replacement components are reviewed. Fractographic investigations generally show that failure modes are completely different from those seen in conventional overload mechanical performance testing, and indicate that subcritical crack growth is often occurring at much lower patient-applied loads than routine testing would suggest could happen. Evidence is seen of the use of inappropriate designs of cone joint between ceramic femoral head and metallic cone, and of surgical mishandling. Localised crack development can occur as a result of handling damage or inappropriate contact with the metallic cone, and under conditions of repeated patient loading can lead to failure after a short implantation time. Key issues in system design and implementation, and in surgical handling, are discussed.
